Double jaw surgery maxillo-mandibular advancement

Double Jaw surgery (Lefort 1 with Bilateral sagittal split osteotomy) can be performed to treat obstructive sleep apnea, malocclusion, and for aesthetic reasons.  The procedure opens the airway by advancing both the upper maxilla and lower jaw.  Both segments can be moved to the best aesthetic and functional position.    The surgery can change the appearance of the face immensely and can improve the facial aesthetics drastically especially in patients with midface and jaw hypoplasia. Obstructive sleep apnea patients who have airway obstruction may benefit from maxillomandibular surgery. A thorough airway evaluation is performed to determine the level of obstruction. If the obstruction appears to be at the skeletal level, maxillo-mandibular advancement will increase the volume of the oropharyngeal airway to treat the obstructive sleep apnea.

Treatment of Vertical Maxillary Excess or “Long face syndrome”

Additionally, Lefort 1 osteotomy can also be used to only advance the top segment and can be used to shorten the midface due to vertical maxillary excess (VME) with removing intervening bone.

Patients with vertical maxillary excess (VME) or “long face syndrome”  can have the vertical height of the maxilla reduced with the LeFort 1 osteotomy. Correction involves decreasing the vertical position of the maxilla and the amount of gingival show.
